Fax Completed Form
Once complete, fax the enrollment form along with a copy of your patient’s insurance card and prescription to XXX-XXX-XXXX. Patients will need to provide verbal consent.

Obtain Patient Consent Via Docusign if Needed
If the patient does not provide verbal consent during enrollment, ensure they provide consent via Docusign to finalize enrollment.

Once enrolled, your patient will receive a welcome call from an (ON)Path Patient Access Advocate explaining the next steps.

Allows eligible patients with certain insurance-related delays to receive a 15-day treatment supply after 3 days.
Quick Start Program
Eligible patients experiencing continued insurance-related delays may receive treatment.

Eligible patients with commercial insurance may pay as little as $0 a month.
Commercial Copay Program
Eligible patients who are uninsured or underinsured and meet other criteria may receive their RevMed treatment at no cost.
